The patient's physical, psychological, and economic acceptance of the wound dressing is also very important. Some dressings may cause skin allergies in some patients. Some patients are not able to accept distinctly smelly wound dressings, such as hydrocolloids and alginates. The comfort and ease of use of a wound dressing become an important reference factor in the selection of a dressing if the patient has high requirements for quality of life. New wound dressings are relatively more expensive, but good dressings can shorten the number of changes. Ultimately, dressings should be used according to the patient's requirements.
Gauze and film type is suitable for first stage healing wounds; film type is suitable for superficial wounds; hydrogel is suitable for wounds with little to moderate exudation; alginate is suitable for wounds with moderate to heavy exudation; hydrogel, hydrocolloid is suitable for dry necrotic wounds.
Since the condition of the wound is constantly changing, the assessment of the wound must be carried out at regular intervals. Depending on the progress of wound healing or the degree of deterioration, it is always possible to assess whether the most appropriate wound dressing has been selected and whether the dressing has been applied correctly.
Hydrocolloid wound dressings are not recommended for wounds with severe infection, exposed bone and tendon, and excessive exudate. Hydrocolloid wound dressings form a gel when in contact with wound exudate, when the dressing is opened, the wound is seen to resemble a septic substance with a special odor, sometimes with a change in color and swelling in appearance, which is the result of the joint formation of the substance and odor of the wound dressing and the protein decomposition in the exudate. Alginate wound dressings can be used for superficially damaged wounds, wounds with moderate to large amounts of exudate, cavities and sinus tracts, infected and bleeding wounds, but are not recommended for dry wounds and wounds with crusts. For wounds with excessive granulation tissue growth and large amounts of exudate, foam wound dressings can be used to keep the area moist and clean with their strong exudate absorption capacity, along with elastic bandages to inhibit granulation tissue proliferation. Keep the wound moist and the surrounding skin dry to avoid affecting the wound repair by changing the wound dressing too frequently. Different types of wound dressings should also be selected according to the grading of the pressure sore, the size of the wound, the presence or absence of infection, the amount of secretion, the degree of pain, the condition of the surrounding skin, and the patient's body position.
